A Supportive Team

Our Medical and Supportive Case Management team helps people living with HIV obtain and sustain the medical care and other core and support services they need to live long, healthy lives. Our Case Managers meet people where they are at and empower them to become willing participants in their own lives and overall care.

We help connect clients to:

  • Primary care physicians

  • Medication and healthcare coverage

  • Housing

  • Therapy, counseling, and other behavioral health services

  • Transportation

  • Dental care

Lastly, we educate clients on the importance of medication and medical adherence for viral suppression.

Did You Know?

Studies have shown that HIV+ clients receiving medical case management are more likely to achieve viral suppression!

Experiencing Intimate Partner Violence?
